Anne, a 49-year-old female, has suffered from weakness all over the body
for two years, accompanied with inconvenient walking for one year. She was hospitalized
in our hospital for further treatment on Sept. 24, 2008.
Essentials for Diagnosis:
1. The patient has suffered from weakness all over the body for two years, accompanied with inconvenient walking for one year.
2. Last Autumn, the patient began to feel numbness and weakness of his left hand’s fingers, and then it gradually spread to the left leg, which made him walk inconveniently.
She was diagnosed with ALS after the MRI examination in a local hospital. Then she got the treatment there, but there was no improvement. His present symptoms were
as follows: weakness all over the body; inconvenient and unsteady walking; weight loss;
obviously physical weakness of the left limbs with decreasing functions of the right limbs;
sometimes dizziness; difficulty in swallowing; spasm and beating of the muscles all over the body; stiffness of the limbs in the morning; sometimes with pains in the joints of the
left side of the body; dry mouth; normal urination and bowel movement.
3. T 36.5℃,P 88 beats/minute, R 20 times/minute, BP 130/86 mmHg.
4. She grew normally with common nutrition. Her mind was clear. She had an expression
of chronic illness and languidness. Her body was in a positive posture and she was
cooperative with clear mind.
5. No thoracic deformity. Sound of breath was bilaterally normal on auscultation. No
sound of pleural friction. Heart border was normal. Heart beat was 88 times/minute.
Cardiac rhythm was regular. No pathological murmurs on auscultation.
6. The muscles of the upper limbs were atrophic, especially serious in the thenar and hypothenar of the left hand. She had difficulty in flexible movement of the fingers.
The gripping power of the left hand was 6.1 kg, and that of the right hand was12.3 kg.
Her tongue was red with cracks in the middle. The tongue coating was yellow. Her
pulse was thready and weak.
7. Diagnostic examination: The MRI shows “ALS”.
Diagnostic Basis:
TCM: The patient has suffered from weakness all over the body for two years, accompanied
with inconvenient walking for one year. His present symptoms were
as follows: weakness all over the body; inconvenient and unsteady walking; weight loss; obviously physical weakness of the left limbs with decreasing functions
of the right limbs; sometimes dizziness; difficulty in swallowing; spasm and beating
of the muscles all over the body; stiffness of the limbs in the morning; sometimes
with pains in the joints of the left side of the body. Her tongue was red with cracks
in the middle. The tongue coating was yellow. Her pulse was thready and weak.
The patient was characterized by limp, weak, and emaciated limbs with muscular
atrophy. Her condition even developed to be unable to stand or walk. So it is not
difficult to diagnose it with wilting.
Western Medicine: The patient has suffered from weakness all over the body for
two years, accompanied with inconvenient walking for one year. The muscles
of the upper limbs were atrophic, especially serious in the thenar and hypothenar
of the left hand. She had difficulty in flexible movement of the fingers. The gripping
power of the left hand was 6.1 kg, and that of the right hand was12.3 kg. The MRI
examination showed “ALS”.
Diagnostic Differentiation:
TCM (Traditional Chinese Medicine):
The patient’s wilting syndrome should be differentiated from impediment syndrome.Wilting syndrome is characterized by limp,
weak, and emaciated limbs with muscular
atrophy. A patient suffered from wilting syndrome seriously may even become unable to hold an object or to stand without
any support. Besides, the patient usually has no joint pains. On the contrary, impediment syndrome is generally characterized by aching pains, fixed heaviness
and
inflexibility of sinews and bones, muscles and joints, with occasional numbness
or swelling, though, no paralytic manifestations exist.
They are not difficult to be
distinguished in clinics.
Western Medicine: Wilting syndrome should be differentiated from syringomyelia, the lesions of which have the obstacles of the separation between the feelings
of pains and touch. That is to say, it is lack of the feeling of pains and exists the feeling
of touch. Wilting and syringomyelia can be distinguished by the MRI examination.
First Diagnosis:
TCM (Traditional Chinese Medicine) diagnosis: Wilting syndrome
Symptom identification: Vacuity of the qi and spleen, yin vacuity of the liver and kidneys.
Western Medicine diagnosis: ALS
Plans for treatment strategy and nursing:
1. Routine care of traditional Chinese internal medicine.
2. Grade II care.
3. Under the care of a companion.
4. Low-fat and high-protein diet.
5. Herbal tea (to boost qi and fortify the spleen, soften the liver and supplement
the kidneys):one dosage a day and drink twice.
Main herbs used in the herbal tea: baisheng (white ginseng), zhimu (anemarrhena root),
gouqi (lycium berry), etc.
6. Acupuncture and massage: once a day.
7. Strengthen the functional exercise of all the limbs.
8. Have more medical examinations if necessary.

The patient has suffered from weakness all over the body for two years, accompanied with inconvenient walking for one year. The muscles of the upper limbs were atrophic,
especially serious in the thenar and hypothenar of the left hand. She had difficulty in flexible
movement of the fingers. She felt pains of the knuckle, wrist joint, left knee joint and ankle
joint. Recently, the patient has lost weight about 8 kg. The muscles all over the body beat
and paroxysmal twitch. She felt dizziness and short of breath with slightly difficulty in
swallowing. There was phlegm when she was coughing. She felt month dry. The tongue was red with cracks in the middle. The tongue coating was yellow. The pulse of the left
hand was thready and weak, and the pulse of the right hand was string-like and slippery.
TCM diagnosis: Wilting.
Symptom identification: Vacuity of the qi and spleen, yin vacuity of the liver and kidneys.
Western Medicine diagnosis: ALS
The TCM treatment strategy is to boost qi and fortify the spleen, soften the liver and supplement the kidneys. Her prescription was as follows: baisheng (white ginseng),
zhimu (anemarrhena root), gouqi (lycium berry), etc. 4 dosages in total.
Date: Sept. 25, 2008 Time: 9:00 a.m.
In her urine analysis, the OBT was + 2. The microscopic examination was abnormal, as she is during the period of menstruation. Her blood routine examination and Chest
Radiography examination were both normal. Besides, her cholesterol was a little high about 7.2. The total calcium was a little low. The other examinations of the functions
of the liver and kidneys were normal.

The patient’s sleep was poor with dizziness. Then the former prescription was changed a little. 5 dosages in total. The doctor told her to supplement calcium and strengthen the
functional exercise. She’d better keep a low-fat and high-protein diet.

Her sleep improved. There was no headache or dizziness. She consciously felt that the strength of the left hand increased. The dry mouth also improved. Her prescription was
changed a little again. 5 dosages in total.
Date: Oct. 7, 2008 Time: 9:00 a.m.
The patient felt the pains in the joints of the upper and lower limbs. The tongue was red with yellow and thick tongue coating. The pulse of the left hand was thready and weak,
and the pulse of the right hand was string-like and slippery. The former prescription
was changed as follows: huangqi (astragalus root), yuanzhi (polygala root),
qinjiao (large gentian root), etc. 5 dosages in total.

The strength of the left limbs increased a lot, and the pains of the joints improved. The stiffness of the knuckle and the flexible movement of the fingers also improved significantly.
She kept the former prescription. 5 dosages in total.
Date: Oct. 18, 2008 Time: 9:00 a.m.
The stiffness and weakness of the left limbs, the pains of the joints as well as the flexible
movement improved significantly. The pains of the knuckle disappeared, so did the pains
of the ankle joint. She kept a sound sleep, a normal diet as well as a pleasant mood. She
was preparing to leave the hospital.
